Caremark Forms and Lists
- Claim Reimbursement Form. For filing claims on drugs a member paid for out of pocket.
- Mail-Order Service Form. For ordering drugs through Caremark's mail-order service.
- Primary/preferred drug list. Helps members find drugs that are covered at the optimal level. Here is a detailed report of recent changes.
- Step Therapy List. Directs members toward generic drugs that must be tried before brand-name equivalents.

- Participating 90-Day Pharmacy List. Helps members find in-network pharmacies that fill 90-day prescriptions.
Under the Medical Plan, charges are eligible for drugs that are (1) intended for use in a physician’s office or setting other than home and (2) billed during the course of an office visit. However, specifically identified oral or injectable drugs that can be self-administered must be obtained through Caremark Specialty Pharmacy.
